Clinical Case Manager - Adults ($3,000 sign-on bonus!)Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Clinical Case Manager to join our talented multi-disciplinary team in our Community Support Program. This is a team that works with young adults and adults who have both a mental illness and an intellectual disability. *This position offers a $3,000 sign-on bonus!As a Clinical Case Manager, your primary responsibilities will be working with clients to provide clinical consultation, case management and therapy services using a number of evidence-based models. You will work closely with treatment teams within Riverbend as well as outside agencies. This is a great opportunity to work on the front lines of community-based behavioral health services, and help our clients achieve optimal recovery and independence.Responsibilities:

  • Individual and group therapy both in the office and in the community
  • Case management and interagency coordination
  • Crisis management
  • Clinical assessments
  • Treatment planning
  • Clinical documentation in accordance with medical necessity guidelines and agency standardsRequirements:
    • Master's degree in Psychology, Social Work, or related field required
    • 1 year experience in a related human service or mental health position preferred.
