Registered Nurse (RN) | Home Health Location: Saint Albans, VT Agency: Cynet Health Pay: $2,766 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/30/2026 About the Position Job Title: Home Health RN Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Tuesday - Saturday Hours per Shift: 8am - 4pm Experience: Minimum 2 years License: Valid nursing license Certifications: CPR certification Must-Have: - Experience with OASIS documentation - Ability to assess patient needs using the nursing process - Case management experience - Strong communication skills Description: Hospice coverage required when on-call. Effectively fills out and completes OASIS documentation in accordance with agency policies and procedures and State and Federal regulations. Demonstrates knowledge of and keeps up-to-date on changes to OASIS documentation requirements as needed. Ensures documentation is completed in accordance with agency practice. Utilizes the nursing process to assess patient and caregiver needs to provide quality care to assigned patients. Develops and communicates a realistic plan of care for each patient. Ensures that the patient and caregivers are included in the care plan process. Serves as case manager for assigned patients and actively manages case loads and visit times. Implements medical plan of care, nursing interventions, and procedures as necessary. Ensures the safe, effective quality of care for patients. Stays up-to-date on a variety of high-tech skills including IV use and maintenance, wound VAC, and complex wound care. Takes action to ensure that infection control standards are maintained throughout the plan of care. Works collaboratively with LPNs to ensure that the plan of care is followed. Delegates tasks and ensures that patient needs are met. Consistently reviews documentation, annotates visit notes, and acts on recommendations. Evaluates aspects of the implemented plan of care including identified problems, care provided, and patient responses. Maintains up-to-date and accurate documentation of nursing care provided. Follows agency protocol with documentation processes. Performs patient and caregiver education to ensure informed patient decisions can be made. Identifies inter-disciplinary needs of the patient and documents them accordingly. Communicates documented information to other disciplines in a timely manner. Accurately fills out a variety of agency, State, and Federal forms in a timely manner. Carries out visits in a time-effective manner. Assesses patients' needs for home care services. Develops and coordinates plans of care with patient and caregivers. Provides ongoing supervision in accordance with regulations. Transmits patient incident reports to the quality improvement specialist as necessary. Coordinates safe patient discharges and/or transfers. Ensures all appropriate paperwork and patient information is processed in accordance with policies and procedures. Coordinates patient care with physicians, providers, team members, and vendors. Participates in monthly peer review audits. Provides constructive feedback and accepts feedback in a professional manner. Schedules, coordinates, and facilitates case conferences with team members.
