qualifications will not be considered. This role focuses on
providing specialized nursing care in a hospital-based skilled
nursing facility that supports geriatric patients recovering after
hospitalization or acute illness. The goal is to help patients
regain independence and prepare for a safe return home, working
closely with a multidisciplinary team to ensure optimal recovery.
Location:The assignment is based in Grafton, WV, within a
hospital-affiliated transitional care unit specializing in
rehabilitative and restorative services. The facility bridges acute
hospital care and home care to support patients in their recovery.
Responsibilities: Deliver skilled nursing care to post-operative
patients (hips, knees, shoulders) and those with conditions such as
pneumonia and congestive heart failure requiring rehabilitation.
Collaborate with physicians, respiratory therapists, pharmacists,
and nurse aides to implement comprehensive patient care plans.
Manage admissions and physician orders under the supervision of the
charge nurse. Participate in a supportive IV team and work
alongside respiratory therapy available 24/7. Maintain compliance
with infection control standards, including a strict no false nails
policy. Provide care within a nurse-to-patient ratio of 1:9-12,
supported by nurse aides and consistent unit orientation.
Qualifications / Desired Experience: Licensed Registered Nurse (RN)
with experience in step-down or transitional care nursing.
Competence in geriatric nursing and rehabilitative care. Proficient
in EPIC documentation system. Ability to work in a 7 AM 7 PM shift
schedule with rotating weekends and holidays. Valid drivers license
required; all submissions must include a copy. Adherence to
facility policies such as scrub color (royal blue) and no floating.
