Job Description:
POSITION TITLE: Advanced Practice Clinician (NP/CNM/PA) LOCATION
: Remote within ME/NH/VT HOURS: Full Time, 30 hours/week, Hourly
(Non-Exempt) UNION MEMBERSHIP: This position is represented by AFT
union in NH /VT & MSEA union in ME POSITION PURPOSE: As an integral
part of health care delivery at PPNNE, the Remote Care Team
provides both Telehealth clinical care to patients, as well as
Centralized Clinical Support to both patients and health center
staff regarding lab work follow up, patient communications,
referral care coordination, and health information management
Affiliate-wide. The Remote Care Team APC will support PPNNE’s
mission of providing accessible reproductive health care through
delivering both high quality, patient-centered, personalized
telehealth visits, as well as daily integration of remote health
care delivery support services to patients and health care teams
Affiliate-wide. YOUR DAY- TO-DAY RESPONSIBILITIES: Provides
exceptional patient-centered care, in a manner that ensures
compliance with current PPNNE efficiency and productivity
standards. Assesses patient's health status and prescribes
appropriate contraceptive methods, treatments for GYN problems
Counsels/educates patients regarding general reproductive health
care; all FDA-approved methods of birth control, including
emergency contraception; pregnancy options including abortion,
adoption as well as Gender Affirming Hormone Therapy Provides
health care that is culturally and linguistically appropriate to
PPNNE patient populations. Performs appropriate clinical care via
telehealth as well as physical exams, with focus on the
reproductive system, including standard initial and annual exams,
and screening procedures Refers patients to physicians and other
health or social services as appropriate Responsible for accurately
coding for medical services provided, in support of the
organization’s overall revenue cycle. Remote Care Team APC’s must
be willing/able to provide in-person care in their home site health
center as needed in addition to providing telehealth care. JOB

Range - for this position is based on years of relevant work
experience. Candidates with 0-6 years of experience can expect to
earn between $ 45.46 - $ 53.64 per hour. As experience increases,
so does the rate of pay, with candidates who have 17 or more years
of direct experience earning up to $72 per hour. BENEFITS: Up to 3

Broad based clinical GYN knowledge and skills Familiarity with and

Excellent counseling/teaching skills Ability to relate to a broad
range of people and communicate clearly, using tact, discretion and
sensitivity with patients Staff providing telehealth remotely must
meet the requirements in the telehealth policy & the requirements
